By submitting this application for the IKORCC JATF (Indiana/Kentucky/Ohio Regional Council of Carpenters Joint Apprenticeship and Training Fund) apprenticeship program, I hereby certify that the information provided in this application is true, complete, and accurate to the best of my knowledge and belief.
