I hereby make application to the Indiana/Kentucky/Ohio Regional Council of Carpenters Joint Apprenticeship and Training Fund.

For the trade of Carpenter, Millwright and Floor Cover:

This Agreement Entered Into This Date:

Between the Indiana/Kentucky/Ohio Regional Council of Carpenters Joint Apprenticeship and Training Fund, hereinafter referred to as the Committee, and

Witnesseth:

That the Committee, the Apprentice, and his/her Parent, or Guardian, desire to enter into an agreement of apprenticeship in conformity with the standards of the United Brotherhood of Carpenters and Joiners of America, and therefore, in consideration of the premises and of the mutual covenants herein contained, do hereby mutually covenant and agree as follows:

That the Apprentice agrees to perform diligently and faithfully the work of said trade or craft during the period of apprenticeship, complying with the training program contained in said Schedule.

That the Parent or Guardian covenants with the Committee that the apprentice will duly perform all obligations undertaken herein.

That this Agreement conforms to, and is predicated upon a local trade agreement on apprenticeship labor standards approved by the United Brotherhood of Carpenters and Joiners of America. The agreement may be annulled by the said local joint committee upon the mutual consent of all parties to the agreement, and in the event of any disagreement or difference in relation to any terms of this Agreement, such disagreement or difference shall be submitted to the local joint apprenticeship committee for adjustment, whose decision shall be final and conclusive upon the parties of this Agreement if supported by the evidence.

That the Apprentice understands and agrees that his or her membership in the United Brotherhood of Carpenters and Joiners of America is subject to termination by the local union or regional council having jurisdiction over enforcement of this Agreement if the joint apprenticeship committee transmits notice to the local union or council that the Apprentice has been dropped from the apprenticeship program for unsatisfactory participation.

Applicants will be required to submit to drug testing.

Pursuant to the policy adopted by the trustees of the Indiana/Kentucky/Ohio Regional Council of Carpenters Joint Apprenticeship and Training Program, all applicants requesting entry into the program must first successfully pass a drug test to be eligible for entry in the apprenticeship program.

Any applicant who does not pass a drug test will not be accepted into the Indiana/Kentucky/Ohio Regional Council of Carpenters Joint Apprenticeship and Training program.

The Indiana/Kentucky/Ohio Regional Council of Carpenters Joint Apprenticeship and Training Committee has adopted an Apprenticeship Loan Agreement. This agreement is a commitment from the Committee that we will provide you with the necessary training, skills, and knowledge to become a qualified Journeyman Carpenter, Millwright, or Floorcoverer. In consideration of this commitment, you agree that you will work as a Carpenter, Millwright, or Floorcoverer for only contractors signatory to a collective bargaining agreement with the Union affiliated with the United Brotherhood of Carpenters and Joiners of America for a period of ten (10) years after completion of your apprenticeship.

As a condition of acceptance into the Apprenticeship Program, you will be required to sign an Apprentice Loan Agreement for that calendar year, and you will also be required to sign a similar agreement for each calendar year of your apprenticeship.

The Apprenticeship Committee will expend a considerable sum of money to provide you with the training to become a Journeyman Carpenter, Millwright, or Floorcoverer. If you do not work for a signatory contractor as defined above, and you remain in the Carpenter, Millwright or Floorcoverer trade, you will be required to repay the amount expended for training on a pro-rata basis for the ten-year period. The repayment schedule and the costs incurred by the Apprenticeship Fund will be set forth in the Apprentice Loan Agreement to which you will become signatory.
